The Fractional Test Meal (FTM) at Pamel Laboratories is a specialized diagnostic procedure used to assess gastric acid secretion and stomach function. It involves the administration of a standardized test meal followed by periodic collection and analysis of gastric juice to evaluate the acidity (pH) and digestive enzyme activity over time.

This test is crucial for investigating gastric disorders such as peptic ulcers, gastritis, achlorhydria, and conditions related to abnormal stomach acid production.

What the Test Measures:

  • Volume, acidity (pH), and appearance of gastric secretions

  • Free and total hydrochloric acid levels at intervals after a test meal

  • Assesses the stomach’s ability to produce and regulate gastric acid

Purpose of the Test:

  • Diagnose or monitor:

    • Hyperacidity or hypoacidity

    • Peptic ulcer disease

    • Chronic gastritis or gastric atrophy

    • Achlorhydria (absence of stomach acid)

    • Zollinger-Ellison syndrome

  • Evaluate unexplained symptoms like:

    • Abdominal pain or bloating

    • Indigestion (dyspepsia)

    • Vomiting or loss of appetite

Procedure Overview:

  1. Fasting overnight before the test

  2. Test meal is administered (usually a stimulant like oatmeal or a mild stimulant of acid secretion)

  3. Gastric contents collected every 15–30 minutes via nasogastric tube

  4. Samples analyzed for volume, acidity, and enzyme content

Sample Type: Gastric fluid (collected through a nasogastric tube)
Fasting Required: Yes (8–12 hours)
Duration: Approximately 1.5 to 2 hours
Turnaround Time: Same day or within 24 hours

Interpretation Notes:

  • Increased acid output may suggest duodenal ulcer or gastrin-producing tumors

  • Decreased or absent acid could indicate gastric atrophy, pernicious anemia, or gastric cancer
